SIR: An examination of the records of this collection district fails to disclose that you have filed an income tax return for the calendar year ended December 31, 1935.

In the event that you filed a return for the year in question, kindly return this notice and answer the questions Nos. 1 to 6, inclusive, listed below.

If you did not file a return, it is requested that you return this notice and answer all of the following questions in order to verify information received concerning your income for the year above-mentioned and to insure a proper check thereof:

How many dependent children under 18 years of age were living with you on December 31st? _ _ _ _ _ 12. Did your wife (or husband) or dependent children receive any income during the year? _ _ _ _ _ If so, how much and from whom? _ _ _ _ _ 13.
