Special instructions:
- Soundex code: PLEASE BE SURE TO INCLUDE THE SOUNDEX CODE. Fields asking for the "Soundex Code" of the bride or groom is referring to the alphanumeric characters found in the upper left and right hand corners of the certificate. Each character begins with a letter. This is the first letter of the last name of either the bride or the groom, which will indicate which person's code it is. In the event that both the bride and groom have last names beginning with the same letter enter the codes, either one in each field, and Archives' staff can verify the codes in their review. Please capitalize the letter.

- Day of ceremony: Please use numbers only- do not include letters such as st, nd , rd, th (1st, 2nd, 3rd, 4th, etc)

- Clerk's handwriting: Wow, quite atrocious in some instances. So much for the beautiful copperplate handwriting we've all heard about. Though it may make for slower going, we very much appreciate your taking the extra time needed to examine the spelling carefully.

- For dates, please use numbers for the day- do not spell them out
