To the Hon. Mayor & City Council

You are hereby notified that
on Saturday evening about 7:30 I was
driving along Illinois Avenue in
the Union Depot Addition to the city of
Fort Worth, near Daggett Street, when
suddenly and without any warning my
horse fell into a deep ditch running
across the street. I was then thrown violently
from the seat of the express wagon onto
and astride of the right from wheel
of the wagon and then & there violently
and seriously injured_ in my neck
& shoulder, in my right groin and
lower stomach, and was then & there
by the fall & lick received in the
fall, ruptured_causeing me
great pain, annoyance and dis-
tress_ to my damage $2500.00 of
which you will take due notice
and that I claim damages against
the city in the said sum, as well as $10.00 for
damages to my express wagon.
