Gentlemen:-
The undersigned respectfully solicits your permision to use the electric light poles of the City Water Works from the Stove Foundry to the Water Works for the purpose of placing a private telephone line thereon.
This line will in no way interfer with the City's lines or wires and we agree to take same down at any time when so requested by the Council or the SUperintendent of the Water Works.

This is to cerify that examined Pink Beckham Cal March 20 1906 and found him suffering from being thrown out of his wagon astride one of the forewheels The condition is one that has to be held in place by a truss Is serious and cant be cured without an operation

You are hereby notified that on Saturday evening about 7:30 I was driving along Illinois Avenue in the Union Depot Addition to the city of Fort Worth, near Daggett Street, when suddenly and without any warning my horse fell into a deep ditch running across the street. I was then thrown violently from the seat of the express wagon onto and astride of the right from wheel of the wagon and then & there violently and seriously injured_ in my neck & shoulder, in my right groin and lower stomach, and was then & there by the fall & lick received in the fall, ruptured_causeing me great pain, annoyance and distress_ to my damage $2500.00 of which you will take due notice and that I claim damages against the city in the said sum, as well as $10.00 for damages to my express wagon.
