That by reason of the premises this claimant files
with the City of Fort Worth this his claim for damages and
alleges the sum to be in the amount of, to-wit: Fifteen-
Hundred ($1500.00) Dollars.

Wherefore premises considered claimant asks that
this his damages be allowed and that he have general and
permanent relief from said injuries as above mentioned.
