Mr. Geo. W. Grandy, Arcadia, California. Dear Sir:- RE: Claim #19049

Under date of August 3rd we were obliged to return your partially completed preliminary notice, and we requested at that time that you complete the same and return it to us at once. Unless we hear from you within the next five days on the subject, we will presume that your ailment was of a very minor nature, and that it not your intention to complete your claim, and will of necessity be obliged to file your claim as not proven.
