Dear Madam Sir:
Under the bonus announcement for 1912, you are entitled to allowance
in the way of credit on your premium receipt book as shown by
enclosed receipt (or receipts).

The receipt should be signed by the insured or by the person who
pays the premiums.

Return bonus receipt (or receipts) with the premium receipt book
and THE POLICY (OR POLICIES): proper entry of the premiums
will be made on the receipt book and policy (or policies), which will
immediately be returned to you.

In order to receive credit for the bonus, the policy must accompany
the receipt.
