Thank you for the advice about using | for making vertical lines. Unfortunately I have disturbed the formatting in the middle of the page, so there are 2 entries without line numbers against them. So this page would need correction. I will try again if it is still open to do so. So sorry...
Great start! However, you don't need to try to format the page as it appears because the system left justifies all of the text. Create the columns to emulate the page, but don't worry about centering text, ditto marks, etc. The system seems to have its own width limit so text will scroll to the next line automatically. You noticed that text in unnumbered lines don't appear in the preview so push columns to the left and make sure each line is numbered. I edited your page so let me know if you have any questions.
