I was unsure about adding the state to place of birth, but since it was outside of Indiana I did so. Is it correct to add the state if it is not Indiana?
Yes, that's exactly right Eden. Even if it is Indiana, please add the state for Place of Birth - there are so many towns and cities that share names from state to state. It isn't necessary for the place of residence, because these are all Indiana residents. - CH
