After transcribing another document similar to this one, I am wondering if what I thought was "A.K" is actually "O.R." and means "Ordered Recorded".
It is probably A.R. (Admitted to Record) as seen on the written statement.
Thanks! I just worked on one that was more clearly "A.R."
