When I use table separators to mark a blank box it seems to hide the entire row (at least in preview) so I have not been following the instruction "If there is no data in a cell, put the table separators with spaces in between."
You only need the fields shown here:

Follow that style and you should be okay with blank fields. Also treat certificate number as a straight numeral
