To the Business Manager:

Application is hereby made for the approval of this detailed statement together
with the plans and Specifications herewith submitted, and for a permit to erect
structure herein described.

The acceptance of the permit herein applied for, shall constitute an agreement
on my part to abide by all the conditions therein contained; and to comply with all
Ordinances of the City of Charlottesville and the laws of the State of Virginia relating
to the work to be done thereunder, and said agreement is a condition of said permit.

Dear Sir:

I have your application for a permit to add to your property on the South s
side of Main Street, wish to advise that before a permit can be issued for the above
mentioned construction it will be necessary for you to comply with Section 221 and
322 of the City Public Safety Act, which in part is as follows: (1) Obtain the consent
of the local Board of Underwriters. (2) Obtain the written consent of the adjacent
property owners, giving them due notice of your proposed improvement. (3) File your
application with the City Engineer, who will present same to the City Commissioners
with his recommendation, at their next regular meeting.

I now have your application and upon receipt of written advice from you that
you have complied with the above mentioned, I will present same to the City Manager
who will bring thematter before the City Commissioners at their next regular meeting.
