The entry option gives two separate blanks for "are you in sound body?" and the following question beginning with "Are you subject to any periodic complaint...?" whereas in the document these two are part of one question with one space for response.
Hi @jts4xq, and thanks for transcribing and for leaving this note! Yes, the second volume of this set has a slightly different setup than the first, and combines those two questions together. Due to the limits of the field setup on this platform, we're just going to have to roll with this! I'll add a note to the transcription conventions that mentions this.
